柳韵普拉提瑜伽工作室免责协议 

1. Introduction 简介

By attending any class or utilizing any service at Willow Flow Studio, you acknowledge and agree to the following liability waiver. This waiver applies to all participants, regardless of whether they are enrolled in a single class, workshop, membership, or other services provided by Willow Flow Studio. 

通过参加柳韵普拉提瑜伽工作室的任何课程或使用任何服务，您确认并同意以下免责条款。本协议适用于所有参与者，无论其是否报名参加单次课程、工作坊、会员卡或普拉提工作室提供的其他服务。 

2. Assumption of Risk 风险承担 

You acknowledge that participation in yoga, aerial yoga, reformer Pilates, and any other physical activity offered at Willow Flow Studio involves inherent risks, including but not limited to physical injury, muscle strain, or other health-related issues. You voluntarily assume all risks associated with these activities. 

您承认，参加瑜伽、空中瑜伽、普拉提核心床以及柳韵普拉提瑜伽工作室提供的任何其他体育活动，涉及固有的风险，包括但不限于身体伤害、肌肉拉伤或其他健康相关问题。您自愿承担与这些活动相关的所有风险。 

3. Medical Considerations 健康声明 

You confirm that you are in good health and have consulted with a physician before participating in any physical activity at Willow Flow Studio. You agree to disclose any known medical conditions, injuries, or limitations that may affect your ability to participate in classes. Please be advised that individuals who are pregnant may not participate in our Reformer Pilates and Aerial Yoga classes. Willow Flow Studio reserves the right to deny participation if it deems that the activity may pose a risk to your health or well-being.

您确认自己身体健康，并在参加柳韵普拉提瑜伽工作室的任何体育活动之前，已咨询医生意见。您同意披露任何已知的健康状况、伤病或可能影响您参加课程的限制。基于安全考量，孕妇将不得参加我们的普拉提核心床和空中瑜伽课程。在柳韵普拉提瑜伽工作室认为该活动可能对您的健康或安全构成风险下，我们有权拒绝您参加活动。 

4. Release of Liability 免责条款 

By attending classes at Willow Flow Studio, you agree to release, waive, discharge, and hold harmless Willow Flow Studio, its owners, instructors, employees, and agents from any and all liability, claims, demands, or causes of action that may arise from your participation in activities at the studio. This release includes but is not limited to any injury, loss, or damage to personal property. 

通过参加柳韵普拉提瑜伽工作室的课程，您同意免除、放弃、解除并保证不会追究柳韵普拉提瑜伽工作室、其所有者、教练、员工及代理人因您参加工作室活动而可能产生的任何责任、索赔、要求或诉讼原因。此免责包括但不限于任何身体伤害、财物损失或损坏。 

5. Indemnification 赔偿条款 

You agree to indemnify and hold Willow Flow Studio harmless from any claims, demands, actions, or expenses (including reasonable attorney’s fees) arising out of your participation in classes or use of the studio facilities. 

您同意赔偿并保护柳韵普拉提瑜伽工作室免受任何因您参加课程或使用工作室设施而引起的索赔、要求、诉讼或费用（包括合理的律师费用）。

6. Emergency Medical Treatment 紧急医疗处理 

 In the event of an emergency, you authorize Willow Flow Studio to seek medical treatment on your behalf and agree to assume responsibility for any medical expenses incurred as a result of such treatment. 

在紧急情况下，您授权柳韵普拉提瑜伽工作室为您寻求医疗处理，并同意承担因此产生的任何医疗费用。 

7. Photo/Video Release 照片及视频发布 

You acknowledge that Willow Flow Studio may take photos or videos during classes for promotional purposes. By signing this waiver, you grant Willow Flow Studio permission to use your image in any media, including social media, without compensation. If you prefer not to be photographed or recorded, please notify the studio in writing. 

您承认柳韵普拉提瑜伽工作室可能在课程期间拍摄照片或视频用于宣传目的。通过签署此免责协议，您授权柳韵普拉提瑜伽工作室使用您的影像于任何媒体，包括社交媒体，无需支付任何报酬。如果您不希望被拍照或录像，请书面通知工作室。 

8. Minors 未成年人的规定 

If you are signing on behalf of a minor (under 18 years of age), you certify that you are the parent or legal guardian of the minor and that you consent to the minor’s participation in classes and agree to all terms of this waiver on their behalf. 

如果您是为未成年人（18岁以下）签署此协议，您证明自己是该未成年人的父母或法定监护人，并同意未成年人参加课程并代表其同意本免责协议的所有条款。 

9. Governing Law 适用法律 

This waiver shall be governed by and construed in accordance with the laws of the jurisdiction in which Willow Flow Studio operates. Any disputes arising from this agreement shall be resolved through mediation or in the courts of this jurisdiction. 

本免责协议应受柳韵普拉提瑜伽工作室运营地法律的管辖，并根据该法律进行解释。由本协议引起的任何争议应通过调解解决，或在本管辖区的法院审理。 

10. Acknowledgment and Acceptance 确认与接受 

By signing this waiver, you acknowledge that you have read and understood the terms outlined above and agree to be bound by them. You understand that by signing this document, you are waiving certain legal rights, including the right to sue Willow Flow Studio. 

通过签署此免责协议，您确认已阅读并理解上述条款，并同意遵守这些条款。您明白，签署此文件即表示您放弃某些法律权利，包括起诉柳韵普拉提瑜伽工作室的权利。 

*By signing up for our class, workshop, membership, or other services provided by Willow Flow Studio—regardless of the platform used to book (such as ClassPass or MindBody)—participants acknowledge that they have read and understood our liability waiver policy *
